Air in the system (ensure boiler is free of air and pressurized to at least 20 PSI); plugged return filter (ensure clear); failed internal circulator/not powered (verify pump operating, blue light on solid, 120VAC when call active); plugged plate heat exchanger (combi)/indirect tank plumbing blockage; improper piping or boiler commissioning (check diverting valve moves freely and is locked in heating position, match piping to the manual for the specific model); damaged/malfunctioning temperature sensor (swap inlet and outlet sensors and verify readings, replace if they differ by more than 5F); safety circuit interrupted (check the loop of black wire, connector 26 on PCB - if a LWCO or other safety is wired into the jumper check the device, reseat the connector and reset).
